Locations: Boston, North Shore and North of Boston

Fortcom Health Services is seeking a dedicated and compassionate Per Diem Registered Nurse (RN) to support our Adult Family Care (AFC) and Adult Foster Care program. This position is ideal for an RN seeking flexible hours while making a meaningful impact through community-based nursing care. 

Position Summary:
The Per Diem RN will conduct in-home visits every two months and complete all required clinical documentation for a limited caseload of assigned members. You will work collaboratively with the Program Director and AFC team to ensure members continue to meet eligibility requirements and receive appropriate care and support in their homes. 

Key Responsibilities: 

  • Complete MDS-HC and other clinical assessments during member visits. 
  • Develop, review, and update individualized AFC plans of care. 
  • Train, evaluate, and supervise AFC caregivers in collaboration with the Program Director. 
  • Review physician summaries and ensure care is consistent with clinical recommendations. 
  • Monitor members’ health status and report changes to their primary care physician. 
  • Conduct admission visits for new members and complete nursing progress notes. 
  • Review AFC caregiver logs periodically to ensure compliance. 
  • Submit semiannual health status reports to members’ primary care physicians. 
  • Plan for and coordinate member discharges from the AFC program as needed. 
  • Provide orientation to new AFC caregivers before the start of care. 
  • Deliver ongoing caregiver training related to health, safety, and aging.
  • Assist with additional office or clinical duties as assigned by the Program Director or COO.

Qualifications:

  • Associate or Bachelor’s degree in Nursing (BSN preferred).
  • Active RN license in good standing with the Massachusetts Board of Registration in Nursing.
  • Previous home health or community-based nursing experience is a plus.
  • Strong clinical documentation and organizational skills. 
  • Bilingual in Spanish or Haitian Creole is a plus.
  • Comfortable with technology and proficient in Microsoft Word, Excel, and PowerPoint.
  • Reliable transportation and ability to travel to members' homes across assigned areas. 

Schedule & Compensation:

  • Per diem hours, flexible scheduling based on availability and member needs.
  • Competitive hourly rate.
  • Mileage reimbursement for home visits.

Compensation: $45.00 per visit
